Who is eligible for Indiana’s Leading Medicaid Home Care Program?
- Have Medicaid
- Live in Indiana
- Need help with day-to-day activities
- Able to direct your own care/services (if not, have an individual representative to appoint)
- 60 and over and qualify for the Pathways MLTSS Waiver, or
- 59 and under and qualify for the Health & Wellness (formerly Aged & Disabled) Waiver
- Someone who is at least 18 years of age
- Someone who eligible for work in the United States
- Someone who is not excluded from the Medicaid program
- The Caregiver may be a non-family member or a family member who lives with the Participant in the private home of the Participant or the Caregiver.

1. Be a Hoosier!
You must live in the State of Indiana.
2. Live with a Caregiver
You must live in the same home as the Caregiver (either their own home or the home of the Participant).
3. Have Medicaid and get on a Waiver
Be eligible for Medicaid or already have Medicaid and meet the financial requirements for the Pathways or Health & Wellness Waiver. (Don’t worry! We can help you!)
